Grad Knight Event Registration
Please follow the following 2-step event registration process to ensure students GradKnight event participation and priority bus seating for the evening of June 12, 2024. Step 1: Complete & Submit the Student Waiver and Step 2: Purchase your Ticket!
Step 1 - Complete & Submit the Student Waiver (s)
Waiver 1A - 2024 Senior Release Form 2024 [Required] and if necessary, the Waiver 1B - the Emergency Care Plan 2024

- Please open REQUIRED Waiver 1A -Senior Release Form 2024 PDF on your computer. You will NOT be able to fill out the from your phone.
- Type the requested information
- Print, sign and email waiver to newportgradknights+waivers@gmail.com. Please include Student Name in the email subject line.
- Please take a moment to review the expectations of the event with your senior on the front page of the senior release. Students will be signing the waiver again at check-in the night of the event.
- Please complete the Waiver 1B - the Emergency Care Plan 2024 [optional] only if medical awareness/accommodations are needed and email waiver to newportgradknights+waivers@gmail.com.
- You will receive an email confirmation from newportgradknights+canned.response@gmail.com if your waiver was received successfully.
- **While electric waiver submission is preferred, your student may return signed forms in an envelope to the counseling center. These will be collected weekly.
